Course content

• Basic principles of moderation of the interdisciplinary FMEA team, differences in moderation of Design FMEA and Process FMEA
• Planning and effective preparation of the moderator for the FMEA team meeting
• Compilation of FMEA team and time schedule of FMEA creation process, i.e. clarification of 5T in terms of the new harmonized manual AIAG & VDA FMEA
• Leading the discussion and monitoring compliance with the 7-step methodology in accordance with the new harmonized AIAG & VDA FMEA manual:
• Step 1: Planning and preparation - identification of the project plan and area of analysis, 5T
• Step 2: Structural analysis - visualization of the analysis object (Structural and block diagrams, process diagrams), identification of subsystems and process steps
• Step 3 - Functional analysis - visualization of functionality (P-diagram, Process flow diagram), a cascade of risks from the manufacturer to the user
• Step 4 - Failure analysis - determination of the failure chain, definition of the failure consequence, failure mode and cause of failure using Fishbone / Cause and effect diagram
• Step 5 - Risk analysis - evaluation of criteria S, O, D and risk according to the priority matrix (AP matrix)
• Step 6 - Optimization - identification of activities necessary for risk mitigation, implementation of corrective measures and their evaluation
• Step 7 - Documenting the results - communication of the results of the management, the customer and the conclusion of the analysis, documenting the corrective measures, including their effectiveness
• Communication techniques and ways to guide the FMEA team during failure analysis, evaluation of criteria and definition of AP matrix level (high, medium, low)
• How to effectively lead a brainstorming team in creating a Fishbone diagram / 5x Why and defining the categorization of disturbances (4M: Man, Machine, Material, Environment)
• Supervision over the documentation of results from the FMEA, evaluation of the effectiveness of the implementation of corrective measures for management
• Common errors and shortcomings in the implementation of FMEA analyzes
• FMEA moderation practice and training with video
• FMEA moderation feedback with video analysis
